Release of Liability
As parent/legal guardian, I give permission for my child to participate in the Anat Baniel
Method®. I understand that Anat Baniel Method® practitioners are teachers, not medical practitioners and the Anat Baniel Method® is an educational modality, not a medical practice or procedure. I agree to release the practitioner and anyone else associated with this practice from any and all claims, causes of action, injuries, damages, costs or expenses arising out of my participation in the Anat Baniel Method®.
Statement of Policy Regarding Intervention Modalities
As you know, the Anat Baniel Method® for Children focuses on movement learning. During the course of providing lessons for your child, we may comment on or answer questions from you or your family members regarding other intervention modalities (such as braces, standers, prescription drugs, a variety of therapeutic modalities etc.) that have proven helpful or obstructive to this particular method of movement learning. At times these comments may appear in conflict with advice you are receiving from your child’s medical professionals. It is important to keep in mind that our suggestions come from a movement learning perspective, while their advice comes from a medical perspective. Every child is unique and only you, having all of your child’s medical and other information, can make the appropriate choices for your child and coordinate medical and other care in a way that advances your child’s best interest. Anat Baniel Method® for Children practitioners are not medical providers and cannot and do not offer medical advice. Where our suggestions appear to be in conflict with the suggestions of your child’s medical providers, it is imperative that you consult with those medical providers and understand from them the consequences and risks of the totality of your child’s care.
Cancellation Policy
I understand that any cancellations must be made at least 24 hours before our appointment.
If I cancel any lessons within the 24-hour period, or after the appointment, I will be responsible
for a payment of $75 for each lesson cancelled.
